Homeport Senior Care is established on the strong foundation that reputable Lucia’s AFH was built for almost 18 years. The home is now managed by An Bui(Anthony), a registered nurse from WSU.
Homeport Senior Care home provides longer term quality care in a home-like atmosphere for your loved ones. Our supportive and family style surroundings promote a respecting and loving environment, while respecting your family member’s personal privacy and dignity.
We Provide:
Medications management, blood sugar monitor, and diabetes care.
* RN as a provider who is on-call 24/7 and nurse delegator visit home every 3 months for any assessment or care plans.
* Dentist, Hygienist, Eyes Doctor, Pharmacist visits as needed.
* Lab work, x-ray, physical therapy, and occupational therapy.
* 24 Hours staffs (Night staff only respond to call button).
* Specialized and licensed in Dementia, Mental Health, Diabetes, Stroke, Parkinson's.
* Daily meals and snacks with special dietary per residents care plan.
* Podiatrist and hairstylist visit ( free for hair stylish ).
* Daily housekeeping and weekly laundry.
* Bathing and personal care.
* Meaningful Day including birthday celebrating, card games, movie nights, music therapy, etc.
* All rooms have call-button, motions detector (if needed), wheel chair accessible, TV with cable, phone and internet connections.
Our home is located in a quiet neighborhood close to shopping pavilions, churches, parks, fire station, hospital and more.
